Description: 1945 Richie Ashburn handwritten questionnaire. Legal sized American Baseball Bureau questionnaire has 19 APR 1945 date stamped at upper corner (exactly 1 month after his 18th birthday). Ashburn signed his name at the top, and added handwritten answers to a number of personal and baseball related questions. All written in ink rating 8 to 8/9 out of 10.
