Shield shaped wooden plaque (13"x10.5") has a metal plate affixed which reads "Outstanding Lithuanian-American Athlete Award Presented To Johnny Unitas, By The St. Casimir's Men's Club Of New Haven, Connecticut, June 7, 1961."
