A metal placard on base front reads in part, "To Tommy Hawkins, Vincent T. Lombardi Hall of Fame Award, In recognition of your outstanding accomplishments in the sports world...Los Angeles Area Council, Boy Scouts of America...November 30, 1993." Hint of light wear. Includes an original program to the event at which Hawkins was honored which was held at Dodger Stadium (wear to program).
