The form lists various items with which they returned including lighters, pearl necklace, two fishing rods, and a small camera. The form has been signed by Joe DiMaggio in blue ink at the bottom of the first page. The top of the front page also lists "Wife-Marilyn DiMaggio" in Joe's hand.
