Original typed questionnaire sent by Syd Pollock on behalf of the Indianapolis Clowns to a 17 year old Henry Aaron in Toulminville, Alabama. The questionnaire is completely filled out by Aaron in fountain pen including a full name "Henry Aaron" signature. Questions responses include: home address, height, weight, former team (Mobile Black Bears), and positions to which Aaron replied "Short Stop or Second base". The final question regarding off season employment was answered with "Grocery Store". Absolutely unique piece dating to the inception of Home Run King Henry Aaron's professional baseball career. Includes LOA from JSA and letter of provenance from the Pollock family: EX-MT
