Description: 1972 Dennis Eckersley handwritten questionnaire. Legal sized questionnaire has Aug. 12, 1972 date stamped at upper corner. Eckersley has handwritten his full name "Dennis Lee Eckersley" at top and handwritten answers to a number of personal and baseball related questions with some choice content. Most notably, he relates his greatest thrill of pitching a shutout in his first professional game. All done in ink rating 8 out of 10. Faint fold lines and a touch of light wear.
