Copy of a statement from former Umpire Dave Phillips relates that he obtained the ball from someone in the clubhouse and attributes game use (ball also incl. COA from PSA/DNA); original full ticket as issued for the game played in Baltimore, likewise signed by Puckett in ink rating 8/9.
